if this is real, then im betting testicular implosion.
 
Well, this is certainly the first time I've ever heard that. Death by masturbation? There can be so many theories to determine the actual cause of death.
 
On a slightly related note,

There's this urban legend of sorts where the lead singer for Three Dog Night had so much sex with women one night that his penis litterally exploded
