He is from Kamen Rider Stronger. He is a member of both Black Satan and the Delza Army. Anyway, this is the most scariest monster IMO in toku:

Nothing beats the super demonic Horrors of Garo in terms of scare level. They had the worst in terms of motive and will. Then this:

That Spider-lady from ZO.... She still gives me the creeps :O_O:
Oh, and as others have said. Some of the demon designs for Garo were quite menacing. There is one though that scared the hell out of me. I am not really sure if it counts, but it is from Episode 12, where we see Koga put on Zaruba for the first time, which brings him to a forest, where he is
chased by an extremely creepy disfigured woman... I guess she counts as one of the Horrors... I hate that scene >_<

Sometimes things get complicated.
Oh wait. . . you said ugliest monster right? Well since I'm not really scared by monsters in movies, I guess I'll pick the ugliest. Ironically enough I'd say that the ugliest one is from the same movie as the stud muffin above.
