Also, this picture actually belongs to Let's Go Kamen Rider, but it says that Ankh was cuffed on this thing. Which means it would be his left arm.

Possibly, but Ankh was possessing Momotaros. They could of cuffed him the other side, but that would be awkward.

Birth's cg gimmick doesn't look too awful. It's definitely better than last year's abomination with the guy who turned into a bike though.
