The "main" forms, like Tatoba, are exceptions that break the naming conventions and don't follow any rules.

The default names for the secondary forms seem to take default names for each medal and mix them together.

Regarding this picture, it says that OOO transformed into RaToraTah and fought the Greeeds 800 years ago. It doesn't seem to be Japan 800 years ago either. Gameru looks slightly different in the picture as well. I'll translate and summarize the rest of the scans if it's needed.

The Yummies created by different Greeeds so far had pretty different results. The physical differences might be justified if the general differences continue happening.
