I just wanted to say that I am making a .xls file with all the OOO combos. Using all the head, chest+arm and leg medals. I am up to 337 different combos. The names might be incorrect. I will upload it on mediafire soon, fixing a few things.

Wow 337 combos?! That I must say is insane. Are you using say, the Hawk Medal for arms and legs too? Cause I'm confused about whether a head specific Medal can be used for other parts as well. Is that what you're doing with your chart?

Heads: Hawk, Stag Beetle, Lion, Rhinoceros, Orca, Pterodactyl, Cobra.
These 7 heads are not used for arms and legs. The arms are not used for heads and legs. The legs are not used for the heads and arms. Since I haven't seen Toei state that head medals can be used for a different part. If Toei say that those 7 medals are usable arms and legs I'll make a new version.
I'll keep updating it as the combo names are confirmed or when new medals are introduced.

339 Combos. Feels like I've missed something. ー indicates a longer sound. That's why there is an "ā" in the Cheetah forms to distinguish it from the Octopus forms (Ta without the "ā") The combo names are just guesses from the sounds that have played by the O Scanner. So they are not official. It's all in Japanese kana, Japanese romaji and English. It's a .xls files with all the combos so far.

I used all the leaked sounds.

Heads: Hawk, Stag Beetle, Lion, Rhinoceros, Orca, Pterodactyl, Cobra.
Arms: Peacock, Mantis, Tiger, Gorilla, Eel, Triceratops, Turtle.
Legs: Condor, Grasshopper, Cheetah, Elephant, Octopus, T-Rex, Crocodile.

Then mixed them all up. The light blue sounds are not used.
