Not sure if it's what you want, but I have a DX Ryuki Survive deck. It has some wear on the chrome, but comes with the following cards:

Survive
Strange Vent
Sword Vent
Shoot Vent
Guard Vent
Trick Vent
Dragranzer
Final Vent

And a Masked Rider Ryuki trading card

All cards are the survive ones. I got it off of StormX a few months ago, but I never got around to getting a belt.

I have an Ouja Deck. But it only comes with a single card, Genocider Advent Card.
